Physical Therapist Assistant

Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist Assistant to deliver compassionate, skilled therapy services to patients in their homes throughout the Lebanon, Missouri area. About the Role

As a Physical Therapist Assistant with Phoenix Home Care and Hospice, you'll work under the supervision of licensed Physical Therapists to provide therapeutic exercises, mobility training, and rehabilitation services to patients recovering from injury, managing chronic conditions, or navigating end-of-life care. You'll visit patients in their homes, adapt treatment plans to home environments, and document progress while maintaining the highest standards of patient care and safety.

Key Responsibilities

• Implement individualized physical therapy treatment plans designed by supervising Physical Therapists
• Instruct patients on therapeutic exercises, proper body mechanics, and techniques to improve strength and mobility
• Assist patients with ambulation, transfers, and activities of daily living to promote independence and safety
• Monitor and record patient progress, pain levels, and functional improvements
• Educate patients and caregivers on home exercise programs and fall prevention strategies
• Maintain accurate, timely clinical documentation and communicate with the care team
• Travel to multiple patient homes within the Lebanon area to provide in-home therapy services

What We're Looking For

• Current Physical Therapist Assistant license in Missouri
• Strong knowledge of therapeutic techniques, mobility assessment, and rehabilitation protocols
• Excellent communication and patient education skills
• Ability to work independently in home settings while collaborating with clinical teams
• Compassion for patients across the care continuum, including hospice and palliative care
• Reliable transportation and flexibility to manage a home visit schedule
• Commitment to patient safety, privacy, and dignity
