RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is hiring a full-time Registered Nurse (RN) Home Health Case Manager in Lee's Summit, Missouri. In this role, you'll provide direct clinical leadership and patient advocacy for our home health population, combining nursing expertise with care coordination to deliver outcomes-driven, patient-centered services.

What This Role Involves

As an RN Home Health Case Manager at Phoenix Home Care and Hospice, you bridge clinical care and care coordination. You'll assess patient needs, develop individualized care plans, manage complex medical situations in the home setting, and collaborate with interdisciplinary teams to ensure continuity of care. You'll advocate for patients and families while optimizing resource use and supporting our mission to enhance quality of life through home-based services.

Day-to-Day Responsibilities

• Conduct comprehensive patient assessments, including medical history, functional status, and psychosocial factors
• Develop and implement individualized care plans aligned with physician orders and patient goals
• Perform skilled nursing care during home visits, including wound care, medication management, and patient education
• Coordinate care with physicians, specialists, social workers, therapists, and community resources
• Monitor patient progress, adjust care plans as needed, and communicate updates to the care team
• Manage caseload documentation, including nursing notes, assessments, and compliance records
• Educate patients and caregivers on disease management, medication adherence, and self-care strategies

Qualifications

• Current RN licensure in Missouri
• Minimum 2 years of nursing experience (home health, community health, or acute care preferred)
• Strong clinical assessment and critical-thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and manage a patient caseload
• Valid driver's license and reliable transportation
• Proficiency with electronic health records and computer systems
