Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist to join our clinical team in Sedalia, Missouri. In this full-time position, you'll deliver compassionate, skilled therapy services to patients in their homes, helping them maintain mobility, manage pain, and achieve their therapeutic goals at the end of life.
What This Role Involves
As a Physical Therapist with Phoenix Home Care and Hospice, you'll provide evidence-based rehabilitation and palliative care in home settings across Sedalia and surrounding areas. You'll work directly with patients, families, and an interdisciplinary care team to develop and implement individualized treatment plans focused on comfort, function, and quality of life. Your clinical expertise will help patients navigate the physical challenges they face, whether that's managing symptom burden, optimizing mobility within their limitations, or preparing the home environment for safer movement and daily activities.
Key Responsibilities
- Conduct thorough physical therapy assessments and establish personalized treatment plans aligned with patient and family goals
- Perform skilled therapeutic interventions including manual therapy, therapeutic exercise, gait training, and fall prevention strategies
- Monitor patient progress, document clinical findings, and communicate updates to the interdisciplinary care team
- Educate patients and caregivers on exercises, energy conservation, mobility aids, and home safety modifications
- Coordinate with physicians, nurses, social workers, and other care team members to ensure integrated, compassionate care
- Maintain accurate, timely clinical documentation in compliance with Medicare, state, and organizational standards
Skills & Qualifications
- Current licensure as a Physical Therapist in Missouri
- Bachelor's degree in Physical Therapy (Master's degree preferred by many employers)
- Valid driver's license and reliable transportation for home visits throughout Sedalia
- Strong communication and interpersonal skills with patients of all backgrounds and functional levels
- Ability to work independently while collaborating effectively within a team environment
- Compassion for end-of-life care and experience with or willingness to learn hospice and palliative care principles
- Clinical experience in home health, rehabilitation, or acute care is valuable
