Registered Nurse (RN) Case Manager, Home Health

Phoenix Home Care and Hospice is hiring a full-time Registered Nurse (RN) Case Manager to join our home health team in Jefferson City, Missouri. In this role, you'll coordinate compassionate, individualized care for patients across our service area, bridging clinical expertise with comprehensive care planning to help people recover and thrive at home.

What This Role Involves

As an RN Case Manager in home health, you are the primary care coordinator for your patient panel. You assess complex medical and social needs, develop evidence-based care plans, communicate with physicians and interdisciplinary teams, and monitor patient progress to ensure continuity of care. Your clinical judgment and organizational skills directly shape outcomes—preventing hospital readmissions, managing chronic conditions, and supporting patients' independence within their own homes.

Your Responsibilities

• Conduct thorough initial and ongoing patient assessments to identify health, functional, and psychosocial needs
• Develop and revise individualized care plans in collaboration with physicians, therapists, social workers, and patients
• Coordinate home visits, communicate clinical findings, and document progress in the electronic health record
• Identify barriers to recovery—medication adherence, mobility, nutrition, family support—and implement interventions
• Act as liaison between patient, family, physician, and care team to optimize communication and resource use
• Monitor for signs of decline or complications and escalate concerns promptly
• Participate in quality improvement, staff education, and team meetings as needed

Skills & Qualifications

• Current, unencumbered Missouri Registered Nurse license
• Minimum one year of clinical nursing experience; home health or case management experience preferred
• Strong assessment, critical thinking, and documentation skills
• Excellent interpersonal and communication abilities with patients, families, and healthcare teams
• Proficiency with electronic health records and basic computer skills
• Reliable transportation and valid driver's license (to travel between patient homes)
• Ability to manage multiple priorities, work independently, and demonstrate compassion under pressure
