Physical Therapist

Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist to join our clinical team in Kansas City, Missouri. As a Physical Therapist, you'll deliver skilled therapeutic care to patients in their homes, helping them achieve mobility, independence, and comfort.

About This Position

This is a full-time role providing physical therapy services to patients throughout the Kansas City, Missouri area. You'll work directly in patients' home environments, developing and delivering personalized treatment plans that complement our comprehensive home care and hospice offerings. Your work will directly improve patient outcomes through evidence-based therapeutic interventions and compassionate, patient-centered care.

Core Duties

• Assess patients' physical function, mobility status, and therapeutic needs
• Develop individualized treatment plans aligned with patient goals and medical status
• Perform therapeutic exercises, manual therapy techniques, gait training, and range-of-motion interventions
• Monitor progress, document outcomes, and adjust treatment strategies accordingly
• Educate patients and caregivers on home exercises, mobility strategies, and fall prevention
• Maintain thorough clinical documentation and ensure regulatory compliance
• Collaborate with nurses, social workers, and interdisciplinary team members to coordinate integrated care
• Adhere to all safety, quality, and organizational standards

What You Bring

• Valid Physical Therapist license in Missouri
• Current CPR/BLS certification
• Strong clinical assessment, treatment planning, and therapeutic skills
• Excellent patient communication and education abilities
• Proven ability to work independently in home-based care settings
• Valid driver's license and reliable transportation
• Commitment to patient dignity, independence, and quality of life
• Professional, organized approach to documentation and compliance
