Registered Nurse (RN) Case Manager, Home Health

Phoenix Home Care and Hospice is seeking a full-time Registered Nurse (RN) Case Manager to join our Home Health team in Overland Park, Kansas. In this role, you'll deliver compassionate, coordinated care that helps our patients thrive in their home environments while collaborating with a dedicated interdisciplinary clinical team.

Role Overview

As an RN Case Manager in Home Health, you'll serve as the clinical hub connecting patients, families, physicians, and support services. Your work centers on comprehensive patient assessment, individualized care planning, and seamless coordination across the care continuum. Your clinical judgment and advocacy directly improve patient outcomes while supporting family caregivers and honoring each patient's preferences and care goals.

Day-to-Day Responsibilities

• Conduct thorough assessments of patient medical history, current health status, functional abilities, and psychosocial needs during skilled home visits
• Develop, implement, and revise individualized care plans in partnership with physicians, therapists, social workers, and family members
• Monitor patient progress toward treatment goals and adjust care strategies based on clinical changes and patient response
• Educate patients and caregivers about medication management, disease processes, self-care, and lifestyle modifications to promote independence and wellness
• Coordinate referrals and maintain communication with specialists, hospitals, community agencies, and insurance providers to ensure continuity of care
• Document clinical assessments, interventions, and progress in a comprehensive, timely manner consistent with regulatory and accreditation standards
• Perform skilled nursing interventions as clinically appropriate within the scope of home health nursing practice

Qualifications

• Current, active Registered Nurse license in Kansas
• Minimum of one to two years of professional nursing experience; home health or case management background strongly preferred
• Working knowledge of home health regulations, Medicare and Medicaid requirements, and accreditation standards
• Strong written and verbal communication skills with ability to document clearly and advocate effectively for patient needs
• Solid clinical assessment and critical-thinking capabilities
• Ability to work independently during home visits while collaborating productively with a team-based care model
• Valid driver's license and reliable transportation for travel throughout the Overland Park service area
• Current BLS/CPR certification required
