Physical Therapist

Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist to deliver specialized rehabilitation and mobility services to patients in their homes throughout the St. Louis, Missouri area. This full-time position allows you to provide direct, patient-centered care in home and community settings while helping individuals regain strength, independence, and quality of life.

What This Role Involves

As a Physical Therapist with Phoenix Home Care and Hospice, you'll work in patients' homes, conducting one-on-one assessments and delivering tailored rehabilitation programs. You'll manage a caseload of patients at various stages of recovery, working closely with interdisciplinary care teams to coordinate comprehensive treatment. Your work combines clinical expertise with compassion, supporting patients through recovery, palliative care, and end-of-life comfort. This role requires reliable transportation and flexibility to visit multiple patient locations throughout the St. Louis region.

Your Responsibilities

• Conduct comprehensive physical assessments and develop individualized treatment plans
• Implement therapeutic exercises, manual techniques, and mobility interventions in patient homes
• Monitor and document patient progress, adjusting treatment approaches as needed
• Educate patients and caregivers on safe movement, home modifications, and self-care strategies
• Communicate regularly with physicians, nurses, and other team members to ensure coordinated care
• Maintain accurate clinical documentation and comply with regulatory and accreditation requirements
• Support patients' goals for independence, comfort, and functional recovery

Qualifications

• Valid Missouri Physical Therapist license
• Bachelor's or Master's degree in Physical Therapy from an accredited program
• Current CPR/BLS certification
• Valid driver's license with reliable transportation
• Strong clinical assessment and treatment skills
• Excellent communication and interpersonal abilities
• Experience in home health, hospice, or acute rehabilitation preferred
