The Opportunity

We're seeking a dedicated Physical Therapist Assistant to provide compassionate, evidence-based rehabilitation services to patients in their homes and community settings throughout the St. Louis area. As a member of Phoenix Home Care and Hospice, you'll work alongside licensed physical therapists to support patient goals, restore function, and enhance quality of life. This is a rewarding role for a skilled PTA committed to delivering skilled therapy in diverse home environments.

What You'll Do

• Perform and document patient assessments, functional evaluations, and progress toward therapy goals under PT supervision
• Implement therapeutic exercises, manual techniques, and evidence-based interventions tailored to each patient's condition
• Educate patients and families on home exercise programs, mobility safety, and fall prevention strategies
• Monitor vital signs and patient responses during treatment; communicate observations to the supervising physical therapist
• Maintain accurate, timely clinical documentation and progress notes for compliance and continuity of care
• Travel to patients' homes and community settings throughout St. Louis to deliver in-home physical therapy services

Requirements

• Current Physical Therapist Assistant license in Missouri
• Graduation from an accredited Physical Therapist Assistant program
• Strong communication skills and ability to build rapport with patients of varying ages and conditions
• Valid driver's license and reliable transportation
• Ability to lift, transfer, and position patients safely; flexibility to work in varied home environments
• Professionalism, compassion, and commitment to patient-centered care
