The Opportunity

Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist Assistant to support patients throughout their rehabilitation journey. Working under the clinical supervision of a licensed Physical Therapist, you'll implement individualized treatment plans, build meaningful relationships with patients, and help them regain mobility, independence, and confidence in their daily activities. This role is essential to our mission of providing comprehensive, patient-centered care in the Eldon area.

Core Duties

• Assist patients with therapeutic exercises, mobility training, and functional activities designed to improve strength, balance, and range of motion
• Implement treatment plans developed by supervising Physical Therapists, documenting progress and reporting patient outcomes
• Provide patient and family education on safe movement, fall prevention, and home safety modifications
• Maintain clean, safe treatment environments and ensure proper care of therapy equipment
• Collaborate with the interdisciplinary care team—nurses, social workers, and physicians—to coordinate seamless patient care
• Travel to patient homes and community facilities within the service area to deliver in-home and outpatient therapy

Skills & Qualifications

• Current Physical Therapist Assistant license or certification in Missouri
• Strong interpersonal and communication skills with the ability to work compassionately with diverse patient populations
• Physical capability to assist and mobilize patients safely, with ability to lift and transfer patients
• Reliable transportation and a valid driver's license
• Proficiency with electronic health records and patient documentation systems
• Ability to work independently in home settings while maintaining accountability to clinical supervision
