RN (Registered Nurse), Home Health

Phoenix Home Care and Hospice is hiring a skilled Registered Nurse for our home health team in Eldon, Missouri. This full-time role brings healthcare directly to patients in their homes, combining clinical expertise with compassionate, personalized care.

Role Overview

As a home health nurse with Phoenix Home Care and Hospice, you'll deliver direct patient care across the Eldon area, conducting in-home assessments, managing treatment plans, and coordinating with physicians and care teams. You'll build meaningful relationships with patients and families while maintaining the highest standards of clinical practice in a dynamic, autonomous environment outside the traditional hospital setting.

Your Responsibilities

• Perform comprehensive initial assessments and ongoing evaluations of patients' medical, physical, and psychosocial needs
• Administer medications, wound care, catheter management, and other skilled nursing interventions as prescribed
• Monitor vital signs and patient progress; communicate changes to physicians and the care team
• Educate patients and caregivers on medication management, treatment procedures, and self-care strategies
• Document care thoroughly in electronic health records to ensure continuity and compliance
• Establish and maintain therapeutic relationships that support patient engagement and outcomes
• Coordinate with social workers, therapists, and other disciplines to address holistic patient needs

What We're Looking For

• Current Missouri RN license in active standing
• Proven experience in home health nursing, hospital nursing, or related clinical settings
• Strong assessment and critical-thinking skills with the ability to work independently
• Excellent communication and patient education abilities
• Valid driver's license and reliable transportation; willingness to travel throughout the Eldon service area
• Proficiency with electronic health records and clinical documentation
• Compassion for patients across diverse backgrounds and conditions
