RN (Registered Nurse), Home Health

Phoenix Home Care and Hospice is seeking a full-time Registered Nurse (RN) to deliver compassionate, skilled nursing care to patients in their homes across Topeka, Kansas. This is an opportunity to make a direct impact on patients' lives while building a meaningful career in home health nursing.

About This Position

As an RN in home health, you'll provide direct patient care and clinical support in patients' residences throughout the Topeka area. You'll work independently and collaboratively, managing patient caseloads, conducting assessments, and coordinating care with interdisciplinary teams. This full-time role combines clinical expertise with the flexibility and autonomy that home-based care offers, allowing you to develop deep relationships with your patients while addressing their evolving health needs in a familiar environment.

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans
• Administer medications, manage wound care, and monitor vital signs and health status
• Educate patients and families about disease management, medication compliance, and self-care techniques
• Document clinical observations and progress in patient records in compliance with regulatory standards
• Communicate patient status to physicians and collaborate with care team members
• Identify changes in patient condition and respond appropriately to ensure safe, quality care

Requirements

• Current Kansas RN license (or equivalent; valid nursing license in state of practice)
• Clinical experience in nursing, with home health experience preferred
• Strong clinical assessment and critical-thinking skills
• Ability to travel independently throughout Topeka and surrounding areas with reliable transportation
• Excellent communication and patient education skills
• Commitment to patient-centered care and professional documentation standards
