About the Role
As a Physical Therapist Assistant with Phoenix Home Care and Hospice, you'll be a vital part of our clinical team in Topeka, bringing direct patient care into homes across the community. You'll implement individualized therapy plans, monitor patient progress, and communicate closely with the PT and broader care team. This position offers the opportunity to make a real difference in patients' lives during critical times—whether supporting rehabilitation after surgery or injury, or providing comfort-focused care in a hospice setting. Your clinical skills, empathy, and reliability will be essential to the quality outcomes our Topeka patients depend on.
Key Responsibilities
- Perform therapeutic exercises and manual techniques under the direction of a licensed Physical Therapist
- Assist patients with mobility, balance training, gait training, and functional activities
- Monitor vital signs and patient response to therapy; report changes or concerns to the supervising PT
- Maintain accurate, timely documentation of patient sessions and progress
- Educate patients and caregivers on home exercise programs and safety
- Transport equipment and supplies; maintain a clean, safe treatment environment
- Collaborate with interdisciplinary teams to support holistic patient care
Who We're Looking For
- Current Kansas state licensure as a Physical Therapist Assistant (PTA) or ability to obtain it
- Graduation from an accredited PTA program
- Proven experience in home health or hospice care (preferred)
- Strong clinical assessment and communication skills
- Ability to lift, bend, and move safely; comfort working in diverse home environments
- Current CPR certification
- Reliable transportation and valid driver's license
- Compassion, patience, and professional integrity
