About This Position

We're seeking a compassionate Occupational Therapist to deliver client-centered care across our Kansas City service area. In this full-time role, you'll conduct occupational therapy assessments, develop personalized treatment plans, and work with patients recovering from illness, managing chronic conditions, or in hospice care. You'll collaborate with our interdisciplinary team to support patients' functional abilities and emotional well-being during critical transitions in their lives.

Core Duties

• Evaluate patients' physical, cognitive, and psychosocial needs to determine occupational therapy goals
• Design and implement treatment plans focused on activities of daily living, mobility, and self-care
• Adapt home environments and recommend assistive devices to maximize patient safety and independence
• Provide therapeutic activities tailored to individual patient goals and diagnoses
• Document patient progress, maintain comprehensive clinical records, and communicate findings with the care team
• Travel to patient homes throughout the Kansas City area to deliver in-home therapy services
• Participate in care conferences and coordinate with physicians, nurses, and other specialists
• Support patients and families with education and coping strategies during recovery or end-of-life care

What We're Looking For

• Valid Occupational Therapist license in the State of Missouri
• Bachelor's degree from an accredited occupational therapy program
• Strong clinical assessment and treatment planning skills
• Experience or genuine interest in home health, hospice, or geriatric care
• Ability to build rapport with diverse patients and families in their home environments
• Excellent communication and documentation skills
• Reliable transportation and willingness to travel throughout the Kansas City service area
• Compassionate approach to patient care and commitment to dignity and independence
