The Opportunity
As an Occupational Therapist with Phoenix Home Care and Hospice, you'll work directly in patients' homes, developing and implementing therapeutic interventions that enable individuals to perform meaningful daily activities. You'll collaborate with interdisciplinary care teams to create personalized treatment plans, teach adaptive strategies, and recommend environmental modifications that restore function and independence. Your clinical expertise will directly improve quality of life for individuals facing serious illness, recovery, or end-of-life transitions in the Sedalia community.
Core Duties
- Conduct comprehensive occupational therapy assessments in patients' home environments
- Design and deliver evidence-based interventions targeting activities of daily living, self-care, mobility, and cognitive function
- Educate patients and family caregivers on adaptive techniques and home safety strategies
- Recommend and train patients on assistive devices and environmental adaptations
- Document clinical findings and progress in the patient medical record
- Communicate with physicians, nurses, social workers, and other interdisciplinary team members
- Maintain current knowledge of occupational therapy best practices in home-based and hospice care
Qualifications
- Bachelor's degree or higher in Occupational Therapy from an accredited program
- Current licensure as an Occupational Therapist in Missouri
- Certification by the National Board for Certification in Occupational Therapy (NBCOT)
- Valid driver's license and reliable transportation for home visits
- Clinical experience in home health or hospice care preferred
- Strong assessment, documentation, and interpersonal communication skills
- Ability to work independently and manage a flexible patient caseload
