Role Overview
As a Skilled Caregiver for Home Care at Phoenix Home Care and Hospice, you'll provide direct, hands-on care to clients with varying health conditions and care needs. You'll be a trusted member of our care team, helping clients maintain independence, dignity, and quality of life while working within their home environment. This is meaningful work that directly impacts the well-being of individuals and families throughout the O'Fallon community and surrounding areas.
Your Responsibilities
- Assist clients with activities of daily living, including bathing, grooming, dressing, and personal hygiene
- Monitor vital signs and report changes in client condition to nursing supervisors
- Provide wound care, catheter care, and other specialized tasks per care plan requirements
- Support medication reminders and proper administration procedures
- Maintain accurate, timely documentation of care provided and client observations
- Communicate effectively with clients, families, and the interdisciplinary care team
- Maintain a safe, clean home environment and assist with light household tasks as needed
- Provide emotional support and companionship to enhance client comfort and well-being
What We're Looking For
- Current certification as a Home Health Aide, Certified Nursing Assistant, or equivalent in Missouri (or eligible to obtain)
- Strong communication and interpersonal skills with clients, families, and healthcare professionals
- Ability to work independently and manage time effectively in home settings
- Physical capability to assist clients with mobility and personal care tasks
- Compassion, patience, and commitment to person-centered care
- Reliable transportation and valid driver's license
- Flexibility to work full-time hours, which may include evening and weekend availability
