The Opportunity
Phoenix Home Care and Hospice is seeking a compassionate, detail-oriented Registered Nurse to serve as a Home Health Case Manager. In this position, you'll combine clinical expertise with care coordination to help patients recover, manage chronic conditions, and maintain independence in their homes. You'll work directly with patients, their families, physicians, and our interdisciplinary team to develop and implement individualized care plans that prioritize quality outcomes and patient satisfaction.
What You'll Do
- Assess patients' medical, social, and emotional needs through comprehensive home visits and detailed patient interviews
- Develop and regularly update individualized care plans based on physician orders and patient goals
- Provide skilled nursing care, including wound care, medication management, and patient education
- Coordinate services across team members—nurses, therapists, aides, and social workers—to ensure seamless care delivery
- Monitor patient progress, document clinical findings, and communicate updates to physicians and the care team
- Advocate for patients' needs and connect them to community resources and support services
- Maintain accurate, timely medical records and ensure compliance with regulatory and agency standards
- Build strong relationships with patients and families to foster trust and engagement in their care
Who We're Looking For
- Current RN licensure in Missouri and valid nursing degree
- Experience in home health, hospice, case management, or acute care nursing
- Strong clinical and critical-thinking skills to assess complex patient needs independently
- Excellent communication and interpersonal abilities with patients, families, and multidisciplinary teams
- Proficiency with electronic health records and medical documentation systems
- Physical ability to conduct home visits and navigate patients' living environments safely
- Commitment to patient-centered care and evidence-based nursing practice
- Valid driver's license and reliable transportation for travel to patient homes throughout Kansas City
