RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is seeking a full-time RN (Registered Nurse) Home Health Case Manager in St. Louis, Missouri, to provide direct clinical oversight and care coordination for patients receiving home-based services. In this role, you'll manage complex patient caseloads and serve as the clinical bridge between patients, families, physicians, and our interdisciplinary team.

About This Position

This is a full-time clinical nursing position based in St. Louis, Missouri. As an RN Home Health Case Manager at Phoenix Home Care and Hospice, you'll combine direct patient assessment with care coordination to optimize outcomes for individuals receiving home health services. You'll work autonomously while maintaining close collaboration with our clinical team, physicians, and specialists to ensure patients' medical, psychosocial, and functional needs are met throughout their care journey.

Your Responsibilities

• Perform thorough nursing assessments for new patients and ongoing evaluations for active cases
• Develop and revise individualized care plans reflecting current medical status and patient goals
• Coordinate care delivery across disciplines including physicians, therapists, social workers, and support services
• Communicate regularly with patients, families, and medical providers to address clinical changes or concerns
• Monitor treatment response and recommend modifications to care strategies based on clinical observations
• Maintain comprehensive, compliant documentation in electronic health records systems
• Identify and arrange appropriate resources, referrals, and community services for patients
• Uphold clinical standards and regulatory compliance in all home health nursing activities

What We're Looking For

• Missouri RN license in active, good standing
• Minimum two years of nursing experience; home health or case management background strongly preferred
• Solid clinical assessment, critical thinking, and problem-solving skills
• Strong interpersonal and communication abilities with patients, families, and healthcare professionals
• Capability to manage multiple cases while prioritizing effectively
• Experience or comfort with electronic health records and medical documentation systems
• Valid driver's license and dependable transportation for patient visits in the St. Louis area
