RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is seeking a full-time Registered Nurse (RN) Home Health Case Manager to join our clinical team in McPherson, Kansas. This position offers the opportunity to provide direct patient advocacy and coordinate comprehensive home healthcare services for individuals across our service area.

What This Role Involves

As an RN Home Health Case Manager with Phoenix Home Care and Hospice, you'll deliver evidence-based nursing care and care coordination in patients' homes. You'll build meaningful relationships with patients and families while managing complex medical needs, coordinating services across multidisciplinary teams, and ensuring each patient receives safe, effective, person-centered care aligned with their goals and medical conditions.

Responsibilities

• Conduct comprehensive nursing assessments in patient homes to identify medical, psychosocial, and environmental factors affecting care
• Develop, implement, and update individualized care plans in collaboration with physicians, patients, and family members
• Manage a diverse caseload of patients with varying acuity levels and diagnoses
• Provide direct nursing interventions, medication education, wound care, and disease management support
• Coordinate referrals and communication with physicians, specialists, therapists, and community resources
• Monitor patient progress, adherence to treatment plans, and clinical outcomes
• Maintain accurate, timely clinical documentation meeting regulatory and company standards
• Educate patients and caregivers on self-management, medication administration, and warning signs
• Advocate for patient needs and ensure quality, dignified care delivery

Skills & Qualifications

• Current Kansas RN license in good standing
• Minimum 1-2 years of home health or related clinical nursing experience
• Strong assessment and critical thinking skills with ability to identify patient safety concerns independently
• Excellent communication and interpersonal skills for working with diverse patients and families
• Proficiency with electronic health records (EHR) and documentation systems
• Valid driver's license and reliable transportation to visit patients throughout McPherson and surrounding areas
• Ability to work independently, manage time effectively, and prioritize complex caseloads
• Compassion, patience, and commitment to patient-centered care
• Flexibility to adjust to varied home environments and patient needs
