The Opportunity

As a Physical Therapist Assistant with Phoenix Home Care and Hospice, you'll work directly in patients' homes, providing skilled therapeutic intervention under the supervision of a licensed Physical Therapist. This role combines clinical expertise with the meaningful work of helping patients regain independence and achieve their rehabilitation goals. You'll be part of a multidisciplinary care team dedicated to patient-centered care in a home-based setting, where your clinical skills make a direct, visible difference in the lives of the people you serve.

Key Responsibilities

• Perform and document therapeutic exercises and mobility activities as directed by the supervising Physical Therapist
• Assist patients with functional training, balance activities, and gait training in the home environment
• Educate patients and caregivers on exercises, safety techniques, and proper body mechanics
• Monitor and report patient progress and any changes in condition to the supervising therapist
• Maintain accurate clinical documentation and treatment records
• Provide safe, respectful care that promotes patient dignity and independence

Requirements

• Current licensure or certification as a Physical Therapist Assistant (PTA), or graduation from an accredited PTA program with eligibility for licensure
• Valid driver's license and reliable transportation for in-home patient visits
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively within a healthcare team
• Physical capability to assist patients with mobility and transfers safely
• Commitment to patient safety, confidentiality, and professional ethics
