Phoenix Home Care and Hospice is seeking a full-time Occupational Therapist to provide skilled therapy services to patients in their homes throughout Joplin, Missouri.

The Opportunity

Join our healthcare team and make a meaningful difference in patients' lives through occupational therapy in the home care and hospice setting. You'll work directly with patients and families, helping them maintain independence, adapt to health changes, and achieve their functional goals during important life transitions.

Core Duties

  • Evaluate patients' functional abilities, living environments, and occupational needs to guide therapy planning
  • Develop and implement individualized occupational therapy treatment plans aligned with patient goals and medical needs
  • Provide skilled therapeutic interventions including adaptive strategies, adaptive equipment training, and functional mobility support
  • Recommend and coordinate environmental modifications and home adaptations to promote safety and independence
  • Document assessments, treatment progress, and clinical outcomes in compliance with regulatory and organizational standards
  • Communicate and coordinate with interdisciplinary team members including physicians, nurses, social workers, and other healthcare professionals
  • Educate patients and caregivers on adaptive techniques, safety practices, and strategies to maintain independence at home

Requirements

  • Master's degree or higher in Occupational Therapy from an accredited program
  • Current Missouri occupational therapy license in good standing
  • Valid driver's license and reliable transportation for visits to patient homes in the Joplin area
  • Strong clinical assessment and treatment planning abilities
  • Excellent communication and collaboration skills with diverse patient populations and healthcare teams
  • Compassionate, patient-centered approach to care in home-based settings
