LPN/RN Private Duty – Full-Time (Pediatric to Young Adult)

Phoenix Home Care and Hospice is seeking a dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our private duty care team in Alba, Missouri. This full-time position offers the opportunity to provide compassionate, specialized nursing care to pediatric and young adult patients in their homes.

Role Overview

As a Private Duty LPN/RN at Phoenix Home Care and Hospice, you'll deliver essential nursing services to individual clients with complex medical and developmental needs. Working directly with families in the Alba, Missouri community, you'll build meaningful relationships while maintaining the highest standards of clinical care. This role combines hands-on patient support with professional nursing judgment, requiring someone who thrives in independent home-based settings while collaborating closely with multidisciplinary care teams.

Responsibilities

• Provide skilled nursing assessment, care planning, and direct patient care aligned with individualized treatment goals
• Administer medications, injections, and prescribed therapies according to physician orders and nursing protocols
• Monitor and document vital signs, patient progress, and health status changes
• Perform wound management, catheter care, feeding tube care, and other specialized nursing procedures
• Assist clients with activities of daily living and mobility support as needed
• Maintain detailed, compliant medical records and communicate updates with families, physicians, and the care team
• Educate families on patient care, equipment use, and health maintenance
• Respond promptly to client needs and address complications in a home environment

What We're Looking For

• Current, active LPN or RN license in Missouri
• Pediatric or developmental disability nursing experience preferred
• Current CPR/BLS certification
• Strong clinical assessment and critical-thinking skills
• Excellent communication and interpersonal abilities
• Reliable transportation and flexibility to work independently in client homes
• Commitment to patient advocacy and family-centered care
