About the Role
As an Occupational Therapist with Phoenix Home Care and Hospice, you'll deliver compassionate, person-centered care to patients in their homes across the Topeka area. This position focuses on helping patients maintain independence, dignity, and quality of life through targeted occupational therapy interventions. Working in home-based care allows you to build meaningful relationships with patients and families while providing specialized therapeutic services tailored to individual needs and goals.
Your Responsibilities
- Conduct comprehensive occupational therapy assessments to evaluate patients' functional abilities and therapeutic needs
- Develop and implement individualized treatment plans focused on activities of daily living, mobility, and independence
- Provide skilled therapeutic interventions using evidence-based practices to support patient recovery and well-being
- Educate patients and families on adaptive strategies, equipment use, and home safety modifications
- Document patient progress and maintain accurate, timely clinical records
- Collaborate with the interdisciplinary healthcare team including nurses, social workers, and physicians
- Communicate with referral sources and coordinate care transitions
Skills & Qualifications
- Bachelor's or Master's degree in Occupational Therapy from an accredited program
- Current licensure as an Occupational Therapist in Kansas
- Professional certification through the National Board for Certification in Occupational Therapy (NBCOT)
- Current CPR/BLS certification
- Strong clinical assessment and treatment planning skills
- Excellent written and verbal communication abilities
- Compassion, patience, and genuine commitment to patient-centered care
- Ability to work independently in diverse home environments
- Valid driver's license and reliable transportation
