Role Overview
As a Speech Language Pathologist with Phoenix Home Care and Hospice, you'll assess, diagnose, and treat communication and swallowing disorders across a diverse patient population throughout the Jefferson City area. This full-time position offers direct clinical impact within an interdisciplinary home care and hospice team, helping patients regain functional communication, maintain dignity, and navigate swallowing challenges during critical transitions in their health journey.
Core Duties
- Conduct comprehensive assessments of speech, language, voice, fluency, and swallowing disorders
- Develop and implement individualized, evidence-based treatment plans
- Deliver direct speech-language pathology therapy in patients' homes and care settings
- Create and maintain detailed clinical documentation and progress records
- Collaborate with physicians, nurses, social workers, and interdisciplinary team members
- Educate patients and families on communication strategies and dysphagia management
- Monitor clinical progress and adjust interventions based on patient outcomes
Qualifications
- Master's degree in Speech-Language Pathology or Communication Sciences and Disorders
- Current Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association
- Active Missouri speech-language pathology license
- Strong clinical reasoning and communication skills
- Ability to work independently and within collaborative care teams
- Valid driver's license with reliable transportation for patient home visits
