Phoenix Home Care and Hospice is seeking a dedicated Occupational Therapist to join our clinical team in Springfield, Missouri. This is a full-time position for a compassionate OT professional ready to make a meaningful difference in patients' lives through in-home therapeutic care and support.

Position Summary

As an Occupational Therapist with Phoenix Home Care and Hospice, you will provide direct patient care in home and community settings throughout the Springfield area. Your role is to help patients regain independence, manage chronic conditions, and improve their quality of life through evidence-based occupational therapy interventions. You'll work collaboratively with interdisciplinary care teams to develop and implement individualized treatment plans that address patients' functional goals and daily living needs.

Day-to-Day Responsibilities

  • Conduct occupational therapy assessments in patients' homes to evaluate functional abilities and identify barriers to independence
  • Develop, document, and implement individualized occupational therapy treatment plans aligned with patient goals
  • Provide hands-on therapeutic interventions addressing activities of daily living, mobility, cognition, and fine motor skills
  • Recommend and train patients and caregivers on adaptive equipment, home modifications, and compensatory strategies
  • Monitor patient progress, adjust treatment plans as needed, and communicate outcomes with the care team
  • Maintain accurate, timely clinical documentation and ensure compliance with regulatory and reimbursement requirements
  • Collaborate with physicians, nurses, social workers, and other clinicians to ensure coordinated, patient-centered care

Skills & Qualifications

  • Valid occupational therapy license in Missouri or licensure eligible
  • Bachelor's degree minimum from an accredited occupational therapy program; Master's degree preferred
  • Ability to travel within Springfield and surrounding areas to provide in-home care
  • Strong communication and patient advocacy skills
  • Knowledge of Medicare, Medicaid, and insurance documentation requirements
  • Proficiency with electronic health records and clinical assessment tools
  • Compassion, flexibility, and commitment to person-centered care
