Speech Language Pathologist – Home Health

Phoenix Home Care and Hospice is hiring a full-time Speech Language Pathologist to deliver direct patient care in Topeka, Kansas. In this role, you'll provide speech, language, and swallowing therapy services to home health patients throughout our service area, helping individuals regain communication and eating abilities while supporting their independence.

About This Position

This is a full-time position focused on providing speech-language pathology services to home-based patients in the Topeka area. You'll work as a member of the interdisciplinary home health team, conducting comprehensive assessments and delivering evidence-based therapeutic interventions within patients' homes. Your work directly impacts quality of life by addressing communication and swallowing disorders across all ages and diagnoses.

Your Responsibilities

• Conduct comprehensive speech and language assessments in patient homes
• Develop individualized treatment plans based on patient needs and medical history
• Provide direct therapy for speech, language, voice, fluency, and swallowing disorders
• Evaluate and manage dysphagia through bedside assessment and diagnostic procedures
• Document patient progress and maintain accurate, timely clinical records
• Educate patients and caregivers on therapeutic techniques and home strategies
• Collaborate with physicians, nurses, and other team members to coordinate patient care
• Comply with all regulations, safety protocols, and quality assurance standards

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program
• Current Certificate of Clinical Competence (CCC-SLP) from ASHA
• Current Kansas state licensure in speech-language pathology
• Valid driver's license and reliable transportation
• Ability to travel to patient homes throughout the Topeka service area
• Strong clinical expertise in speech, language, cognitive, and swallowing disorders
• Excellent communication and patient education abilities
• Experience in home health, hospital, or outpatient clinical settings preferred
