About This Position
This full-time weekend on-call position offers Registered Nurses the opportunity to deliver skilled hospice care to patients approaching end of life. Based in Topeka, Kansas, you'll be part of a compassionate care team committed to comfort, dignity, and quality of life during life's final chapters. The role combines direct patient care with clinical coordination across Phoenix Home Care and Hospice's service area.
Your Responsibilities
- Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual comfort needs
- Develop and implement individualized hospice care plans in collaboration with interdisciplinary care teams
- Manage symptoms, pain, and medical complications using evidence-based palliative care practices
- Administer medications, monitor vital signs, and document clinical findings accurately and thoroughly
- Communicate with physicians, families, and care team members to ensure coordinated, patient-centered care
- Respond to urgent calls and provide skilled nursing interventions on nights, weekends, and holidays as scheduled
- Support family members with education, emotional support, and guidance during difficult times
Who We're Looking For
- Current Registered Nurse (RN) license in good standing in Kansas
- Minimum two years of nursing experience, with preference for hospice or palliative care background
- Strong communication skills and compassion for patients and families experiencing life-limiting illness
- Ability to work independently and make clinical decisions in dynamic, home-based settings
- Reliable transportation within the Topeka area; current driver's license required
- Comfort with on-call availability and flexible weekend, evening, and holiday scheduling
- Certification or willingness to obtain hospice-specific training
