RN (Registered Nurse) Hospice Case Manager

Phoenix Home Care and Hospice is seeking a compassionate, skilled Registered Nurse (RN) to join our team as a Hospice Case Manager in Springfield, Missouri. In this full-time role, you'll provide expert clinical leadership and coordinated care for patients and families navigating end-of-life care, delivering personalized support while ensuring compliance with hospice standards and best practices.

About the Role

As an RN Hospice Case Manager, you'll be the clinical anchor for our patients' care journeys. You'll assess complex medical and psychosocial needs, develop individualized care plans, and guide patients and families through one of life's most meaningful transitions. Working within a multidisciplinary hospice team, you'll combine clinical expertise with genuine compassion to honor each patient's wishes and dignity.

What You'll Do

• Conduct comprehensive patient assessments and establish person-centered care plans aligned with hospice philosophy and regulatory standards
• Coordinate care across nursing, social work, chaplaincy, and volunteer services to address physical, emotional, and spiritual needs
• Provide direct clinical guidance, medication management oversight, and symptom monitoring for assigned patient caseload
• Communicate with patients, families, and healthcare providers to facilitate informed decision-making and manage expectations
• Document clinical findings and progress notes to maintain accurate, compliant medical records
• Participate in interdisciplinary team conferences and quality improvement initiatives

Requirements

• Active Missouri Registered Nurse (RN) license in good standing
• Clinical nursing experience, with hospice or end-of-life care experience preferred
• Strong assessment and critical-thinking skills with ability to manage complex patient presentations
• Excellent communication and interpersonal abilities; sensitivity to diverse cultural and spiritual perspectives
• Proficiency with electronic health records and ability to work independently and collaboratively
• Valid driver's license and reliable transportation for in-home patient visits
