RN (Registered Nurse) Home Health Admissions

Phoenix Home Care and Hospice is hiring a full-time Registered Nurse (RN) to lead patient admissions and initial clinical assessments in our Kansas City, Missouri office. In this role, you'll be the first clinical point of contact for new home health patients, ensuring each admission is thorough, compliant, and sets the foundation for excellent individualized care.

About the Role

As an RN Home Health Admissions specialist at Phoenix Home Care and Hospice, you'll conduct comprehensive initial patient evaluations and develop the clinical framework for individualized home health services in Kansas City. You'll work closely with physicians, families, and our interdisciplinary care team to ensure smooth transitions into home-based care and establish clear expectations for each patient's treatment plan. This position combines advanced clinical expertise with strong communication skills—you'll help patients and families understand their care options during a critical time.

Your Responsibilities

• Perform complete patient intake assessments, including medical history, current medications, and functional status evaluation
• Develop individualized home health care plans in collaboration with ordering physicians and care team members
• Educate patients and families about home health services, equipment, and self-care protocols
• Document all admissions information accurately and ensure compliance with state and federal regulations
• Communicate clinical findings and care recommendations to attending physicians and nursing leadership
• Coordinate with scheduling and field staff to arrange timely first home visits
• Identify and escalate complex medical or social needs requiring specialized interventions

What You Bring

• Active RN license in the state of Missouri
• Two or more years of nursing experience (home health, acute care, or community health preferred)
• Strong clinical assessment skills and ability to prioritize multiple patient evaluations
• Excellent verbal and written communication abilities
• Knowledge of home health regulations, compliance, and documentation standards
• Proficiency with electronic health records (EHR) and computer systems
• Patient-centered approach and ability to work collaboratively with interdisciplinary teams
