RN (Registered Nurse) Case Manager, Hospice

Phoenix Home Care and Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our hospice team in Overland Park, Kansas. In this full-time position, you'll provide comprehensive care coordination and nursing expertise to patients and families navigating end-of-life care.

Role Overview

As an RN Case Manager in our hospice program, you'll serve as a clinical leader and patient advocate, connecting individuals with the care, resources, and support they need during this significant time. Working within our interdisciplinary team, you'll develop and implement individualized care plans that prioritize comfort, dignity, and quality of life for each patient and their loved ones.

Key Responsibilities

• Conduct comprehensive assessments of patients and families to identify clinical, emotional, and spiritual needs
• Develop, implement, and maintain individualized care plans that reflect patient goals and preferences
• Coordinate care delivery across nursing, social work, chaplaincy, volunteer services, and other clinical disciplines
• Monitor patient symptoms and clinical status; communicate changes and updates to physicians and team members
• Provide patient and family education on comfort care, medication management, and available supportive services
• Maintain accurate and timely clinical documentation
• Serve as an advocate for patient and family needs and wishes

Skills & Qualifications

• Current Registered Nurse (RN) license valid in Kansas
• Clinical nursing experience, preferably in hospice, palliative care, or acute care environments
• Strong clinical assessment and critical thinking abilities
• Excellent verbal and written communication skills
• Ability to collaborate effectively within multidisciplinary team settings
• Compassion and emotional resilience when working with patients and families
• Strong organizational and time management skills
