The Opportunity

We're building a care team that puts patients and their families first. As a caregiver or CNA with Phoenix Home Care and Hospice, you'll work directly in patients' homes, providing essential support and building meaningful relationships with the people you serve. This role offers the flexibility and purpose that comes with hands-on healthcare work in a home-based setting.

Key Responsibilities

• Provide personal care assistance including bathing, dressing, grooming, and toileting
• Support patients with mobility, transferring, and positioning to prevent injury
• Monitor and report changes in patient condition to the clinical team
• Assist with light housekeeping and meal preparation as part of patient care
• Maintain accurate patient records and communicate clearly with supervisors
• Follow established care plans and respond to patient needs with professionalism and compassion

What You Bring

• Current CNA certification or equivalent experience in patient care
• Strong communication and interpersonal skills
• Ability to work independently and follow clinical protocols
