LPN/RN Pediatric Home Health Nurse

Phoenix Home Care and Hospice is hiring a full-time LPN/RN Pediatric Home Health Nurse to deliver compassionate, evidence-based care to children in the El Dorado, Kansas area. This is an opportunity to make a direct difference in young patients' lives while working in the home setting.

Role Overview

As an LPN/RN Pediatric Home Health Nurse with Phoenix Home Care and Hospice, you'll provide skilled nursing care to pediatric patients within their home environments. This role combines clinical expertise with the personal connection that home-based care offers, allowing you to build meaningful relationships with families while addressing each child's unique health needs.

Responsibilities

• Deliver direct patient care, including vital sign monitoring, medication administration, wound care, and other skilled nursing interventions as prescribed
• Assess pediatric patients' health status and develop individualized care plans aligned with physician orders
• Educate patients and families on health conditions, medication management, and self-care techniques
• Coordinate with interdisciplinary care teams including physicians, therapists, and social workers
• Maintain accurate, timely clinical documentation and medical records
• Respond with compassion to patient and family concerns in the home environment

Skills & Qualifications

• Active licensure as an RN or LPN in Kansas, with current CPR certification
• Experience in pediatric nursing or demonstrated competency in caring for children
• Strong clinical assessment and communication abilities
• Ability to work independently and manage multiple patients across different locations
• Reliable transportation and flexibility with scheduling
• Compassion for patients and families, with a patient-centered approach to care
