The Opportunity

Phoenix Home Care and Hospice is seeking a dedicated LPN/RN to provide essential pediatric nursing services across our Bellefontaine Neighbors service area. As a pediatric home care nurse, you'll play a vital role in improving health outcomes for young patients by delivering skilled nursing directly where families are most comfortable—in their own homes. This role combines clinical expertise with compassionate family support, offering meaningful work with a manageable day and weekend schedule in the greater St. Louis region.

Key Responsibilities

• Assess and monitor pediatric patients' medical conditions and vital signs during home visits
• Administer medications, injections, and treatments as prescribed by physicians
• Perform wound care, dressing changes, and other clinical procedures
• Educate families on patient care, medication management, and health maintenance
• Maintain detailed clinical documentation and communicate patient progress to the care team
• Collaborate with physicians, therapists, and social workers to coordinate comprehensive care plans
• Respond promptly to patient and family concerns during scheduled visits

Who We're Looking For

• Current Missouri LPN or RN licensure in good standing
• Demonstrated experience in pediatric nursing or home health care
• Strong clinical assessment and communication skills
• Ability to work independently during home visits while collaborating with the care team
• Compassionate approach to working with children and supporting families
• Reliable transportation and flexibility for day and weekend scheduling
