LPN/RN Pediatric Home Care

Phoenix Home Care and Hospice is seeking a compassionate and skilled LPN/RN to deliver exceptional pediatric home care services in Washington, Missouri. This is a full-time position where you'll make a direct difference in the lives of children and their families.

Role Overview

As a Licensed Practical Nurse or Registered Nurse specializing in pediatric home care, you'll provide direct clinical nursing support to children with acute, chronic, and complex medical needs in their home environments. This role requires clinical expertise, compassion for young patients, and the ability to work independently while collaborating with interdisciplinary care teams. You'll be the trusted healthcare professional families rely on during some of their most challenging moments.

Responsibilities

• Deliver direct patient care including wound care, medication administration, catheter care, and monitoring of vital signs
• Assess pediatric patients' physical and emotional well-being, documenting observations and changes in condition
• Educate families on care procedures, medical equipment operation, and recognizing signs requiring clinical intervention
• Collaborate with physicians, case managers, and family members to adjust care plans as needed
• Maintain accurate, timely clinical documentation and ensure compliance with regulatory standards
• Provide direct physical care including hygiene assistance, feeding support, and mobility assistance
• Respond to patient needs and escalate concerns to supervisory staff when appropriate

Skills & Qualifications

• Current LPN or RN licensure in Missouri
• Demonstrated experience or training in pediatric nursing
• Ability to assess and respond to complex pediatric medical conditions
• Strong communication skills with both families and clinical teams
• Reliable transportation to travel between patient homes throughout Washington and surrounding areas
• Compassion, patience, and professional demeanor when working with children and families
• Ability to work independently and manage time effectively in home-based settings
