RN Home Health, Weekend On-Call Admissions

Phoenix Home Care and Hospice is seeking a dedicated Registered Nurse for our RN Home Health, Weekend On-Call Admissions position in Springfield, Missouri, United States. This is a full-time role where you'll provide essential clinical leadership during weekend admissions and on-call support.

Role Overview

As an RN Home Health professional focused on weekend on-call admissions, you'll be a critical part of our clinical team. Your expertise in patient assessment and intake will directly impact the quality of care our home health patients receive in the Springfield area. This role combines the clinical judgment of a Registered Nurse with the flexibility and patient-centered focus that home health demands, with specific emphasis on weekend coverage and new patient admissions.

Responsibilities

• Conduct thorough patient assessments and clinical intake evaluations for new home health admissions
• Develop individualized care plans based on patient needs and clinical findings
• Provide weekend on-call coverage for new patient admissions
• Document all clinical interactions and care decisions in accordance with regulatory standards
• Communicate effectively with patients, families, and interdisciplinary team members
• Ensure compliance with home health regulations and best practices
• Coordinate care with physicians and other healthcare providers
• Respond promptly to clinical questions and concerns from the home health team

What We're Looking For

• Active RN license in Missouri
• Experience or strong interest in home health nursing
• Proficiency in patient assessment and clinical evaluation
• Excellent written and verbal communication skills
• Ability to work independently and make sound clinical judgments
• Flexibility to support weekend on-call admissions schedules
