LPN/RN Homecare (Children to Young Adults)

Phoenix Home Care and Hospice is seeking a compassionate and skilled Licensed Practical Nurse or Registered Nurse to provide direct homecare services to children and young adults in Macks Creek, Missouri. The Opportunity

As an LPN or RN in our homecare program, you'll work independently and collaboratively to support patients' health goals, manage complex medical needs, and build lasting relationships with families. This role combines clinical expertise with the flexibility and personal satisfaction that comes from community-based healthcare delivery.

Your Responsibilities

• Provide skilled nursing assessments, care planning, and direct patient care to pediatric and young adult patients in their homes
• Monitor vital signs, administer medications, manage medical equipment, and perform wound care and other clinical procedures as appropriate
• Maintain accurate, timely documentation of patient progress and communicate findings with the interdisciplinary care team
• Educate patients and caregivers on disease management, medication administration, and self-care strategies
• Respond to changes in patient condition and coordinate with physicians and other healthcare providers
• Support family-centered care by building trust and collaborating with parents, guardians, and household members

Skills & Qualifications

• Current LPN or RN licensure in Missouri
• Experience or comfort working with pediatric and adolescent populations in clinical settings
• Strong communication and interpersonal skills with patients and families
• Ability to work independently while maintaining communication with the care team
• Reliable transportation and flexibility to travel within Macks Creek and surrounding areas
• Commitment to infection control, patient safety, and professional standards of nursing practice
