About the Role

As a Hospice Admissions RN with Phoenix Home Care and Hospice, you'll be on the front lines of patient care, conducting comprehensive assessments and establishing the foundation for compassionate end-of-life support. Key Responsibilities

• Perform thorough nursing assessments during patient admissions, documenting medical history, current symptoms, and care needs
• Educate patients and families about hospice services, goals of care, and what to expect during the admission process
• Collaborate with physicians, social workers, and interdisciplinary care teams to develop individualized care plans
• Complete all required admissions documentation, consent forms, and medical records in compliance with regulatory standards
• Establish initial relationships with patients and caregivers, providing emotional support and answering questions
• Communicate with referring providers and healthcare facilities to coordinate continuity of care
• Maintain accurate, timely clinical documentation using electronic health record systems

Who We're Looking For

• Current Missouri Registered Nurse (RN) license in good standing
• Nursing experience in healthcare settings; hospice, palliative care, or acute care experience preferred
• Strong assessment skills and ability to gather comprehensive patient health information
• Excellent communication abilities, both written and verbal
• Compassion for patients and families facing end-of-life decisions
• Professional, organized approach to managing multiple admissions and documentation
