What This Role Involves

As an RN Case Manager in our Home Health division, you'll serve as the clinical and care coordinator for a diverse patient population across the Branson area. You'll conduct thorough patient assessments, develop individualized care plans, and ensure seamless communication among physicians, specialists, therapists, and support staff. Your work directly improves patient outcomes by preventing hospital readmissions, managing chronic conditions, and helping patients navigate their healthcare journey with confidence and clarity.

Your Responsibilities

• Perform comprehensive nursing assessments and establish evidence-based care plans aligned with physician orders
• Coordinate care across all members of the interdisciplinary team, including physicians, home health aides, and therapies
• Educate patients and families on disease management, medication adherence, and self-care strategies
• Monitor patient progress through regular home visits and telehealth follow-ups; adjust care plans as needed
• Manage insurance authorizations, pre-approvals, and documentation to ensure continuity of care
• Maintain detailed, compliant clinical records and communicate status updates to referring providers
• Advocate for patient needs and address barriers to optimal health outcomes

What You Bring

• Current RN licensure in Missouri
• Minimum 2+ years of nursing experience (home health or case management experience preferred)
• Strong clinical assessment and critical-thinking skills
• Excellent written and verbal communication abilities
• Proficiency with electronic health records (EHR) and comfort learning new systems quickly
• Reliable transportation and valid driver's license
• Compassion, patience, and genuine commitment to patient-centered care
