About the Role

As a Registered Nurse – Heart Failure Coordinator at Yale New Haven Health's Waterbury location, you will provide direct patient care and disease management education while coordinating care across clinical departments. Reporting to the Nurse Manager and Clinical Coordinator, you will combine clinical nursing expertise with care coordination to help patients understand their condition and achieve their health goals. What You'll Do

  • Assess and monitor heart failure patients, evaluating clinical status and responding to changes in condition

  • Schedule follow-up appointments and maintain communication with patients regarding their treatment plans
  • Support medication management education and reinforce treatment adherence strategies
  • Collaborate with cardiologists, physicians, and interdisciplinary team members on care planning
  • Maintain accurate, timely documentation in electronic health records
  • Advocate for patient needs and preferences throughout the care continuum

What We're Looking For

  • Current Connecticut Registered Nurse (RN) license
  • Strong clinical assessment and interpersonal communication abilities
  • Prior experience in acute care, cardiac nursing, or care coordination preferred
  • Ability to work effectively as part of a collaborative healthcare team
  • Genuine commitment to patient-centered care and quality outcomes
