LPN/RN Pediatric Home Care Weekends

Phoenix Home Care and Hospice is seeking a dedicated LPN/RN for Pediatric Home Care on a full-time weekend schedule in St. Peters, Missouri. What This Role Involves

As an LPN/RN Pediatric Home Care nurse, you'll provide direct patient care to children in their homes, working weekend shifts. You'll combine clinical nursing skills with the flexibility and patient focus that home care demands, building relationships with families while managing pediatric patients' complex medical needs in a non-hospital setting.

Key Responsibilities

• Provide direct nursing care to pediatric patients in home settings, including assessments, medication administration, wound care, and vital sign monitoring
• Collaborate with interdisciplinary care teams, physicians, and families to develop and adjust care plans
• Document patient information, progress notes, and care activities in accordance with medical records standards
• Monitor and report changes in patient condition promptly
• Educate families on care procedures, medication management, and health maintenance
• Maintain infection control and safety protocols in home environments
• Support end-of-life care when applicable, demonstrating compassion and clinical expertise

What We're Looking For

• Current, active licensure as an RN (Registered Nurse) or LPN (Licensed Practical Nurse) in Missouri
• Experience or proven interest in pediatric nursing or home care nursing
• Strong clinical assessment and communication skills
• Ability to work independently and build trust with patients and families
• Reliable transportation and availability for weekend shifts
• Commitment to infection control and patient safety best practices
• Compassion and patience when working with children and their caregivers
