RN (Registered Nurse) Home Health Case Manager

Phoenix Home Care and Hospice is seeking a full-time Registered Nurse (RN) Case Manager to deliver comprehensive care coordination and clinical oversight in our home health services across Wichita, Kansas. This role combines clinical nursing expertise with care management to ensure patients receive coordinated, patient-centered care in their own homes.

About the Role

As an RN Home Health Case Manager at Phoenix Home Care and Hospice, you'll be the clinical leader coordinating care for a caseload of patients receiving home health services. You'll assess patient needs, develop individualized care plans, communicate directly with physicians and interdisciplinary team members, and serve as the primary point of contact ensuring continuity of care. This position requires strong clinical judgment, compassionate patient advocacy, and the ability to work independently in community-based settings throughout the Wichita area.

Responsibilities

• Perform initial and ongoing patient assessments, documenting clinical findings and identifying care needs
• Develop, implement, and update individualized care plans in collaboration with physicians and the care team
• Coordinate services among nurses, therapists, social workers, and other home health providers
• Communicate with patients, families, and referring physicians regarding patient progress and care adjustments
• Maintain accurate, timely documentation in the electronic health record system
• Travel to patient homes throughout Wichita for assessments and follow-up visits

Skills & Qualifications

• Current Kansas Registered Nurse (RN) license
• Minimum experience in nursing, preferably including home health or case management
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities with patients, families, and clinical teams
• Proficiency with electronic health records and computer systems
• Valid driver's license and reliable transportation
• Knowledge of HIPAA regulations and commitment to patient privacy
