What You'll Do
- Evaluate patients' physical conditions and develop individualized treatment plans to restore mobility and reduce pain
- Deliver evidence-based therapeutic interventions in patient homes and care environments across Moberly
- Assess patient progress, adjust treatment protocols, and document skilled therapy services to regulatory standards
- Educate patients and caregivers on therapeutic exercises, proper body mechanics, and safety strategies
- Collaborate with physicians, nurses, social workers, and care team members to ensure coordinated, holistic patient care
- Maintain detailed, timely documentation to support quality care and billing compliance
- Communicate effectively with patients, families, and the care team regarding treatment goals and progress
Requirements
- Current Missouri state license as a Physical Therapist or eligibility to obtain one
- Proven clinical experience in home care, hospice, or skilled rehabilitation settings
- Strong assessment and treatment skills across orthopedic, neurological, and geriatric rehabilitation
- Excellent communication and interpersonal abilities with patients, families, and multidisciplinary teams
- Reliable transportation and valid driver's license for in-home patient visits in the Moberly area
- Ability to manage time effectively and prioritize multiple patient caseloads in a dynamic environment
