Physical Therapist

Phoenix Home Care and Hospice is seeking a dedicated Physical Therapist to join our team in Moberly, Missouri. As a full-time Physical Therapist, you will provide compassionate, skilled rehabilitation services to patients in their homes and care settings throughout Moberly and the surrounding region.

About the Role

We're looking for a licensed Physical Therapist committed to improving patient mobility, function, and quality of life through evidence-based rehabilitation. In this role, you'll deliver skilled physical therapy services within a patient-centered home care and hospice model, working closely with interdisciplinary teams to support patients through their healing and comfort journeys. What You'll Do

• Evaluate patients' physical conditions and develop individualized treatment plans to restore mobility and reduce pain
• Deliver evidence-based therapeutic interventions in patient homes and care environments across Moberly
• Assess patient progress, adjust treatment protocols, and document skilled therapy services to regulatory standards
• Educate patients and caregivers on therapeutic exercises, proper body mechanics, and safety strategies
• Collaborate with physicians, nurses, social workers, and care team members to ensure coordinated, holistic patient care
• Maintain detailed, timely documentation to support quality care and billing compliance
• Communicate effectively with patients, families, and the care team regarding treatment goals and progress

Requirements

• Current Missouri state license as a Physical Therapist or eligibility to obtain one
• Proven clinical experience in home care, hospice, or skilled rehabilitation settings
• Strong assessment and treatment skills across orthopedic, neurological, and geriatric rehabilitation
• Excellent communication and interpersonal abilities with patients, families, and multidisciplinary teams
• Reliable transportation and valid driver's license for in-home patient visits in the Moberly area
• Ability to manage time effectively and prioritize multiple patient caseloads in a dynamic environment
