Registered Nurse (RN) – Home Health – FT

VitalCaring Group is seeking a full-time Registered Nurse to deliver high-quality home health care in South Hutchinson, Reno County. This position offers the clinical independence and patient connection that make home-based nursing uniquely rewarding—with direct responsibility for managing patient care plans, building therapeutic relationships, and driving measurable health outcomes in your community.

About the Role

As a Registered Nurse in our Home Health program, you'll practice independent clinical nursing in a field-based setting where your decisions directly improve patient lives. You'll evaluate complex patient needs, implement evidence-based interventions, and coordinate care across disciplines—exercising the full scope of your license while working autonomously to make a real difference.

What You'll Do

• Conduct comprehensive patient assessments, develop individualized care plans, and adjust interventions based on clinical judgment and patient response
• Perform skilled nursing care including wound management, medication management, patient education, and chronic disease monitoring
• Communicate with physicians, social workers, and family members to ensure coordinated, person-centered care
• Document clinical findings, outcomes, and patient progress in compliance with regulatory standards
• Travel to patient homes throughout the South Hutchinson and Reno County area to deliver care and build lasting therapeutic relationships
• Advocate for patient needs and contribute to quality improvement initiatives

What We're Looking For

• Current RN licensure in Kansas with an active, unrestricted license
• Strong clinical foundation with ability to assess complex patients independently
• Excellent communication and patient education skills
• Reliable transportation and a valid driver's license for field-based work
• Commitment to patient-centered care and evidence-based practice
• Home health or acute care nursing experience preferred
