Position Summary

As a Private Duty Care Coordinator, you'll serve as the central point of contact for care quality and team efficiency. Your role bridges patient needs with caregiver resources, ensuring personalized, consistent care in our clients' homes throughout the Springfield area. You'll coordinate schedules, manage documentation, communicate with families, and work collaboratively with clinical staff to deliver excellent private duty care services.

Day-to-Day Responsibilities

  • Schedule and coordinate caregiving staff for client assignments, ensuring adequate coverage and continuity of care
  • Respond to patient and family inquiries, addressing concerns and updates about their care plans
  • Maintain accurate and complete client records, care notes, and service documentation
  • Communicate with nurses, physicians, and other healthcare professionals regarding client status and care adjustments
  • Conduct initial client assessments to understand care needs and preferences
  • Monitor care quality by conducting follow-ups with clients and care staff
  • Handle administrative tasks including billing support, paperwork processing, and compliance documentation
