RN Home Health, Weekend Admissions

Phoenix Home Care and Hospice is seeking a Registered Nurse (RN) for our Home Health weekend admissions team in Overland Park, Kansas. Join our clinical team as a full-time RN home health professional, specializing in patient intake and weekend admissions in the greater Kansas City area.

What This Role Involves

As an RN Home Health professional focused on weekend admissions, you'll be at the front line of patient care, conducting thorough intake assessments and establishing care plans for new patients entering our home health services. This is a patient-facing position that combines clinical nursing expertise with the organizational skills needed to onboard patients smoothly into our care system. You'll work within a supportive clinical team and help set the tone for quality care from a patient's first visit forward.

Day-to-Day Responsibilities

• Perform comprehensive nursing assessments during patient home visits on weekends
• Complete intake paperwork and establish initial care plans aligned with physician orders
• Collect vital signs, patient history, and relevant clinical data
• Educate patients and families about their care plans and home health services
• Coordinate with our clinical team and interdisciplinary care coordinators
• Document assessments and findings in our electronic health record system
• Communicate clinical findings to physicians and care managers
• Maintain compliance with home health nursing standards and regulatory requirements

What You Bring

• Current RN licensure in Kansas
• Experience in home health nursing or acute care with comfort working in patient homes
• Strong assessment skills and ability to identify patient needs quickly and accurately
• Excellent communication and patient education abilities
• Ability to work independently on weekend shifts with strong time management
• Reliability and commitment to supporting our admissions process
• Proficiency with electronic health records systems
• CPR certification and current knowledge of infection control practices
