RN (Registered Nurse), Home Health

Join Phoenix Home Care and Hospice as a full-time Registered Nurse (RN) in our Home Health program in Columbia, Missouri. We're looking for a dedicated RN to provide skilled nursing care to patients in their homes, helping them recover, manage chronic conditions, and maintain independence.

What This Role Involves

As a full-time RN in our Home Health team, you'll deliver evidence-based nursing care directly to patients in their homes throughout Columbia and surrounding areas. You'll manage a caseload of patients with varying acuity levels, building meaningful relationships while ensuring clinical excellence. This is an opportunity to make a tangible difference in patients' lives by providing compassionate, skilled care in the most familiar and comfortable environment.

Key Responsibilities

• Complete comprehensive patient assessments, including vital signs, health history, and current medical status
• Develop and implement individualized care plans based on physician orders and patient needs
• Perform skilled nursing procedures including wound care, catheter management, and IV therapy administration
• Educate patients and families on disease management, medication administration, and self-care techniques
• Monitor medication compliance and therapeutic responses; report changes to the care team
• Maintain accurate, timely documentation of patient visits and clinical findings in compliance with regulations
• Coordinate with physicians, case managers, and interdisciplinary team members to optimize patient outcomes
• Identify and respond promptly to changes in patient condition or potential safety concerns

Skills & Qualifications

• Current, unencumbered Registered Nurse (RN) license in Missouri
• Minimum of 1 year of clinical nursing experience
• Strong assessment and critical thinking skills
• Excellent written and verbal communication abilities
• Ability to work independently and manage time effectively
• Compassionate patient care philosophy
• Reliable transportation and valid driver's license
• Proficiency with electronic health record (EHR) systems
