LPN/RN Pediatric Home Care

Phoenix Home Care and Hospice is hiring a full-time LPN/RN to deliver specialized pediatric home care services in O'Fallon, Missouri. You'll provide compassionate, skilled nursing to children with complex medical needs, working closely with families and healthcare teams to support health and wellbeing in home settings.

What This Role Involves

As a pediatric home care nurse, you'll serve as the primary healthcare provider for children requiring ongoing medical support in the O'Fallon area. You'll manage treatment plans, monitor clinical progress, and adapt care to each patient's unique developmental needs. This full-time position emphasizes direct patient and family relationships, clinical independence, and the clinical rewards of home-based pediatric nursing.

Day-to-Day Responsibilities

• Perform nursing assessments and monitor vital signs, medical status, and treatment response for pediatric patients
• Administer medications, manage feeding tubes, wound care, and other prescribed therapeutic interventions
• Maintain comprehensive medical documentation and communicate patient updates to physicians and care teams
• Educate families on condition management, medication administration, safety protocols, and emergency procedures
• Coordinate with specialists, social workers, and other providers to ensure integrated, family-centered care
• Respond to medical changes and implement appropriate clinical interventions and emergency protocols
• Maintain current licensure, certifications, and professional development standards

What You Bring

• Current, unrestricted RN or LPN license in Missouri
• Experience in pediatric home health, acute care, or related clinical nursing settings
• Excellent communication skills with children, adolescents, and families
• Ability to work independently while adhering to clinical protocols and safety standards
• Reliable transportation and valid driver's license
• Compassion, adaptability, and a family-centered approach to care
