What You'll Do
- Conduct thorough patient assessments to develop individualized physical therapy treatment plans
- Deliver therapeutic interventions including therapeutic exercises, manual techniques, and functional mobility training
- Monitor patient progress, document outcomes, and adjust treatment protocols based on clinical findings
- Collaborate with nursing staff, occupational therapists, and social services to ensure coordinated care
- Educate patients and families on exercises, adaptive strategies, and discharge planning
- Maintain detailed clinical documentation and communicate findings with the healthcare team
What We're Looking For
- Valid state physical therapy license in Vermont
- Bachelor's degree in Physical Therapy (DPT or equivalent)
- Current CPR certification
- Minimum 1–2 years of clinical experience in physical therapy
- Experience in skilled nursing facility or acute care settings preferred
- Strong clinical judgment, communication, and patient interaction skills
- Ability to work effectively as part of a multidisciplinary team
