About the Role

The Case Manager role centers on breaking the cycle of homelessness by walking alongside clients through every stage of their housing journey—from street outreach through permanent housing placement. What You'll Do

  • Conduct detailed intake assessments and collaborate with clients to create Individualized Service Plans (ISP) that address housing, income, health, and personal goals
  • Connect clients with essential resources, benefits, healthcare, mental health services, and other supports needed to reach their ISP objectives
  • Complete clinical assessments such as the VI-SPDAT to understand client needs and eligibility
  • Check in with clients after 90 days to evaluate progress, adjust plans, and celebrate wins
  • Maintain thorough, confidential case documentation and files according to HIPAA standards and program requirements
  • Build meaningful professional relationships that keep clients engaged and motivated toward housing goals
  • Respond to crises with clear risk assessment and de-escalation, teaching clients effective coping strategies and problem-solving skills
  • Stay current with local resources and partner agencies to offer wrap-around services tailored to each person's situation

Essential Requirements

  • Valid work authorization in the United States
  • CPR and First Aid certification (or willingness to obtain it)
  • Reliable personal vehicle and valid driver's license—you'll visit clients and community locations throughout the service area
  • Proof of vehicle insurance and ownership
  • Must pass a background screening, tuberculosis test, and drug screening before hire
