PATH (People Assisting the Homeless) is recruiting a Case Manager for our Supportive Housing Services team in Los Angeles County. In this full-time role, you'll work Monday through Friday from 8:00 AM to 4:30 PM providing intensive case management support to individuals in stable housing who are rebuilding their lives after experiencing homelessness.

About the Role

As a Case Manager with our Intensive Case Management Services (ICMS) program, you'll work within our permanent supportive housing sites—including our The Six apartments in Downtown Los Angeles—connecting clients with the tools, services, and networks they need for long-term stability. You'll be the primary point of contact for residents as they navigate housing maintenance, employment, health services, financial planning, and community connection. This position requires reliable transportation, client-centered commitment, and a proactive approach to problem-solving.

What You'll Do

  • Conduct comprehensive intake interviews and strength-based assessments with new residents to understand their goals, barriers, and support networks
  • Develop and maintain individualized service plans that address housing, income, health, mental health, substance use recovery, employment, and social integration goals
  • Connect clients with community resources, treatment providers, employment programs, financial assistance, and social services to support their objectives
  • Administer specialized assessment tools to inform service planning and track client progress
  • Meet regularly with clients (including structured check-ins at 90 days) to evaluate progress, adjust plans, and recognize achievements
  • Maintain thorough, accurate, and timely case file documentation while ensuring full HIPAA compliance and client confidentiality

We operate prevention, outreach, employment, and permanent supportive housing programs across the state. Our Supportive Housing Services programs deliver intensive case management within permanent housing settings, enabling individuals with chronic health conditions, mental health challenges, or substance use histories to achieve stable housing and improved quality of life.

Requirements & Compensation

  • Experience: Two years in case management, healthcare, homeless services, or work with vulnerable populations is preferred; however, all experience levels are welcome to apply.
  • Qualifications: U.S. employment authorization required. CPR and First Aid certification required; if you don't have it, you must be willing and able to obtain it.
  • Background & Health Screening: Candidates must successfully complete tuberculosis testing, background screening, and drug screening as a condition of employment.
  • Driving & Transportation: Reliable personal transportation is essential to this role. You must possess a valid California driver's license, proof of vehicle insurance and ownership, and the ability to qualify for PATH's insurance coverage.
  • Compensation: $23.24 to $28.28 per hour, based on experience. We offer medical, dental, and vision coverage; vacation and sick leave; paid holidays; and a retirement plan.
