About the Role

As a Case Manager on our Housing Navigation Team, you'll deliver comprehensive, client-centered support that addresses immediate shelter needs alongside enduring housing solutions. Working from our Los Angeles location in a Monday through Friday, 8:00 a.m. to 4:30 p.m. schedule, you'll build meaningful relationships with clients that drive measurable outcomes. This on-site position requires reliable personal transportation, as field visits to clients and community organizations are central to the work.

What You'll Do

  • Meet with clients to understand their situation and develop personalized service plans that address housing, income, health, mental health, substance use, vocational, financial, and social support needs
  • Identify and connect clients with community resources, benefits programs, and support services through direct referrals and advocacy
  • Administer validated assessment tools (including the VI-SPDAT) to evaluate vulnerability and determine service priorities
  • Review client progress at structured intervals, evaluate goal achievement, and adjust service plans accordingly
  • Maintain thorough, accurate case documentation and client records in compliance with HIPAA and PATH policies

  • Valid driver's license, reliable personal vehicle, proof of auto insurance, and ability to qualify for PATH insurance coverage
  • Willingness to obtain CPR and First Aid certification; ability to pass background screening, tuberculosis test, and drug screening prior to hire
